PRI at the Evolution of Addiction Treatment Conference
Do you know most researchers define alcoholism differently than most counselors? How confusing when we try to be research based. Do you know the DSM-IV diagnostic label "Dependent in Full Sustained Remission" can be applied to those still drinking, and according to a recent national study about 30% do so and succeed? And what about the new subtypes of alcoholism suggested by NIAAA researchers? They include a subtype that occurs when young and then goes away. That may be confusing, but have you ever known someone who seemed to have alcoholism, but many years later was drinking without problems? Finally, most counselors work from the paradigm that alcoholism and addiction are characterized by loss of control and are irreversible. Yet our primary diagnostic tool, the DSM-IV, makes no such assumptions. Are you sure DSM-IV Dependence diagnosis means the same thing you mean when you diagnose "alcoholism" or "addiction"?
A shift has occurred in the paradigms that are central to our work; yet it has gone largely unnoticed and uncommented on. This unconscious shift creates conflicts we may not see unless we stop to reflect. Once we do we may never see dependence and addiction in the same way again - and out clients may thank us for it.
